Yes. Every resident receives an individual care plan tailored to their medical needs, preferences, routines, mobility requirements, dietary needs, and personal interests. Care plans are regularly reviewed and updated by the nursing team.
A care plan is created before a resident comes to us. We work with you, your loved one and any relevant medical professionals (GPs, physiotherapists etc) to ensure we cover all needs. We put particular emphasis on working with the resident to ensure our care plan matches their personal preferences and reflects their likes and dislikes.
All care plans are flexible and regularly reviewed to ensure they reflect the residents current level of care required. Family are encouraged to take an active part in these reviews.
